It is a condition whereby fat deposits in a disproportionate manner - mainly in the lower torso. It does not respond to weight loss or exercise.
It is a fairly common condition among women and can present from puberty on-wards. Certain life events can spur it on such as childbirth, weight gain, menopause and lifestyle changes.
We also think that genetics have a certain role to play.
In most cases patients present with the complaint that they cannot lose fatty bulk from the waist down. Often the upper torso responds well to weight loss and exercise. Patients thus often have a "pear" shape. Occasionally some fatty deposit may also present on the upper arms.
The condition can remain stagnant after a specific time or it can progress to much more complex stages at which stage the limbs become very heavy and difficult to move. There may be excess skin and folds.
There are 3 stages.
Conservative measures include MLD (manual lymphatic drainage), bandaging, wearing compression hosiery and keeping and eye on your dietary intake. Regular exercise also helps.
Interventional methods include tumescent liposuction and VASER liposuction. Both these methods remove the excess tissue.
Removing the excess tissue is only part of the treatment. Once the bulk has been removed maintenance treatment with compression garments and MLD massage are essential.
Research has shown that the earlier one reduces the bulk the less likely the condition will progress.
